In agreeing to these Terms & Conditions, I affirm that I have read, accept and understand this course includes both educational and exercise style content. I know that there may be risks associated with fitness classes and willingly accept those possibilities. I know that it is my responsibility to ensure my own health and safety. I take full responsibility for my participation and to the extent I deem advisable, will consult with my physician before participating in any of the activities.

I understand that this course is not physical therapy or a substitute for individualized physical therapy care. This means that diagnosis and treatment is not included in this course and that all information provided is for general education purposes only. I understand that I am responsible for monitoring my own condition throughout the course materials. Should any unusual symptoms occur, I will cease my participation and inform my doctor.

In consideration for being allowed to participate in this activity, which I do freely and voluntarily for my own personal benefit, and have been cleared by my doctor to participate in exercise, I hereby waive, release and discharge any and all liability to Pieton Physical Therapy Inc.
